INFORMED CONSENT AND LIABILITY RELEASE

I understand the nature of the event and give my permission for my son/daughter’s voluntary participation in the Annual Easter Egg Scramble. I release the sponsoring organizations and permit holder from any liability involved from my son/daughter’s involvement from my son/daughter’s participation in this program. This includes injury of any type and I understand the risks associated with a group easter egg hunt.

MEDIA CONSENT

There will be a videographer and photographer present during the event and your child may have the opportunity to have his/her picture/video taken and included on one of the sponsoring organization’s website. This includes the CityPlace social media channels, as well. I do give my permission for my child to participate in media activities that will take place during the Annual Easter Scramble on Sunday April 9th, 2023 between 11am-4pm at Canoe Landing Park.

YOUTH CODE OF CONDUCT

• Absolutely no bullying, horse play or aggressive physical contact is allowed during the Easter Scramble by parents and any participating children.
• All participants are expected to follow instructions and behave in a respectable manner.
• No cursing or screaming is tolerated.
• All kids must only race in the quadrant of their specified age group.
